Referral Process

Referrals are most commonly initiated by a short term acute care hospital's discharge planner. Discharge planners include; Social Workers and Case Managers.

However, anyone can make a referral to InterMedical Hospital of SC. In order to be admitted to IMH a physician order is required.

The discharge planner should fax the patient's demographic sheet along with the IMH Referral Check Sheet. If you do not have a referral check sheet please call the Admissions Coordinator at (803) 296-3757 to obtain one.

The Admissions Coordinator will complete the Insurance Verification Form and the Clinical Liaison will complete an on-site Pre-Admission Assessment.

All clinical documentation will be reviewed. A determination will be made to whether a potential client will benefit from a clinical program offered at IMH.
